Template Confidentiality Agreement for Specialists

Counselling sessions with me me are confidential. However, if I have concerns about any information in respect to serious risk to self or others; or you lead me to believe that you or someone else have committed an unreported very serious criminal offence - then I reserve the right to break confidentiality. I will always try to discuss these issues with you within your session and advise that confidentiality will be broken.

As a practicing counsellor I do consult a supervisor and selected, anonymised information may be discussed with them. This is part of my commitment to the Code of Ethics required by the British Association for Counsellors and Psychotherapists (BACP).

I have read and understood the confidentiality agreement.
